When the permit no longer matches the job
HSE describes permits as a control and communication system. A signature does not authorise changed conditions.
Understand the situation
A permit records agreement for a defined task under defined conditions. The same crew and equipment can face different hazards when the method, timing or surrounding plant changes.
Match the authorised scope to the actual work.
Review the task
Suspend affected work and maintain a safe state when a change is identified. Describe the change to the permit authority.
Operations and interacting crews assess the effect, then use the site’s amendment or reissue process.
Check your workplace
- Did scope or timing change?
- Did adjacent work or plant conditions change?
- Who confirms suspension, reassessment and reauthorisation?
Apply the lesson
For example: removing bolts becomes cutting a pipe. Sparks, heat and combustible exposure now need assessment.
The label “pipe replacement” does not automatically cover the new method.
Avoid a common mistake
Before restart, verify physical controls as well as the revised document. Record the change, verifier, conditions and time so the next shift understands what remains authorised.

A permit depends on its conditions
A signed permit remains meaningful only while its scope and conditions remain applicable. A different equipment item, work method or adjacent activity can invalidate the original assumptions.
Describe what changed, which hazards it introduces and which controls still apply. Do not assume the existing permit covers the changed task before reassessment.
