Record a decision, not just attendance
Safety communication must be usable by the next person. HSE handover principles can inform follow-up after a meeting.
Understand the situation
Minutes should show what will change, as well as who attended. “Keep access tidy” leaves the action and completion criteria unclear.
Review the task
Give each decision an action, owner, deadline and verification method. Separate completion from effectiveness and specify interim controls.
Photography is only one recording method and must follow security rules.
Check your workplace
- What change was agreed?
- Who owns it and by when?
- Who verifies effectiveness?
Apply the lesson
For example: move materials to the designated storage area, verify clear passage before the next task, and retain the restriction if unfinished. This is not a statutory minute template.
Avoid a common mistake
Begin the next meeting with follow-up. If the condition recurs, examine why materials return or storage is inadequate rather than only repeating reminders.

Example: remove stored materials from access route; named owner; before work; verify passage; keep restriction if incomplete. Follow site security rules for photos.

Record decisions through to field completion
Discussion does not demonstrate implementation. Without an acceptance criterion, a purchase order or photograph can be mistaken for removal of the hazard.
For a blocked route, define completion in terms of usable access and prevention of renewed obstruction. Record temporary arrangements separately from the permanent correction.
