Handing over isolation information

HSE highlights preparation, information exchange and cross-checking by the incoming shift. ‘Under maintenance’ is insufficient isolation detail.

Understand the situation

Isolation remains in force when a shift changes. Information known by the outgoing person is not automatically known by the incoming one.

Confirm equipment identity, work limits and the isolation-list revision together.

Review the task

Prepare incomplete work and unresolved conditions before handover. Compare the same list and use a cross-check by the incoming person.

Transfer or remove locks only through the authorised site procedure.

  • Which equipment and isolation points?
  • What work, people and tools remain?
  • Who controls removal and restart?

Instead of “pump under maintenance,” an example records the task, list revision, unfinished waste recovery and no-restart restriction. The example identifiers do not refer to actual plant.

Avoid a common mistake

Acceptance of a handover is separate from maintenance completion and restart approval. Identify who checks remaining work, personnel and tools, reinstatement and authorisation.

Suggested fields: job ID, isolation-list version, lock custodian, residual hazards, next check and handover time. Follow the authorised lock-transfer procedure.

Technical insight

Maintain ownership of isolation at handover

Progress alone is not enough for handover. The incoming shift needs to know who controls each isolation and which unfinished tasks still depend on it.

Link equipment identification, lock control, open work and restrictions on reinstatement. Possession of a key alone does not establish authority to remove protection.
